User details: complete name and surname, PERSAL or staff ID, existing username, region or municipality, library or office and job title or assignment.
Reason: select Transfer, New duties, Reduce rights or Correction, as appropriate.
Current access: record the current role and libraries. Under Remove, identify the old role or library access to be removed.
Grant: record the proposed role and libraries needed for the new duties. Describe the work and its effective date. Add an end date for temporary access.
Authorisation: obtain the line manager confirmation and Library Services approval with names, signatures and actual dates. A privileged exception goes to GITO review as required by the form.
Office use: the authorised implementer records old rights removed and new rights granted. An independent verifier records the before-and-after evidence, verifier and date.
